For FSA and HSA, eligible expenses include medical, dental, vision, prescriptions, and approved over-the-counter items. Refer to our Eligible Items page, IRS Publication 502, or your plan’s Summary Plan Description for details. HRA plan designs and eligible expenses vary, so please refer to your specific plan documents to learn more. For Dependent Care FSA, eligible expenses include care provided for eligible dependents while you and your spouse (if married) work, look for work, or attend school full-time.

For most benefits and services, you will need an itemized receipt or Explanation of Benefits showing date of service, service description, patient name, and out-of-pocket cost. For Dependent Care FSA, you will need an itemized receipt from the provider that includes the provider’s name and Tax ID, dates of service, description of service, dependent name, and the amount owed.

Most claims are reviewed within a few business days. If your request is approved and funds are available, your reimbursement will be issued shortly after.

By default, reimbursements are mailed as a check to your home address. To receive your funds faster, you can set up direct deposit by logging in to your FloresHR participant account. You can also check the status of your reimbursement there anytime.

Log in to the FloresHR portal to request a replacement or additional card for dependents. The first replacement is free.
