Flores collects and uses information about you if you are enrolled in a benefit plan administered by one of our customers, if you sign up for or open a health-related financial account with one of our customers, or if you are a direct consumer of one of our products. In these situations, Flores may collect the following types of personal information to provide our Services to you:
- Basic Personal Information and Personal Identifiers Names, including your own name, the name of any dependents covered by your account, the company you work for, relationship to a person as well as personal identifiers, user ids and social security numbers. There may be circumstances where additional information or documentation is required to verify your identity, such as driver’s license number or passport number is required.
- Demographic and Contact Information: Postal and mailing addresses, phone number, age, marital status, and gender information.
- Commercial and Financial Information: Services purchased by you and your consumer history, bank account information or other pertinent billing or payment information necessary to process financial transactions necessary to facilitate our services.
- HIPAA and Non-HIPAA Covered Health Information: Medical information associated with medical bills, health insurance information, and any information necessary for a claim for reimbursement
- Internet Activity and Location: The Websites usage history, login information, IP address and geolocation data
- Sensory Data: Audio recordings of calls
- Employment Information: Employer and employment history
- Educational Information: Student information such as student loan repayment information
Flores collects these types of information from one or more of the following sources:
- On account forms, or in communications with us
- From your employer, health plan, or enrollment provider
- Information from public sources such as government records
- From information you provide us on our Websites, or over the phone
- From information viewing our Websites
- From third party service providers that assist with providing these benefits
Flores may collect or disclose the above information for the following purposes:
- To provide you with information about our Services, or to administer our Services, including determining your eligibility to receive such Services
- To review and pay claims
- To evaluate and provide additional benefits, or to provide information about existing benefits, to you or any person designated by you to receive such information
- To ensure service providers can assist in providing our Services to you
- To perform analytics on our products’ performance, to enhance a product or service, or to enhance the Websites experience
- To comply with the law, including responding to subpoenas or inquiries from regulatory agencies or law enforcement
- For purposes allowable or required under HIPAA and other applicable regulations
Sharing and Disclosure of Your Information
Flores shares and discloses information with third parties to perform services on behalf of us, or for you. We require our service providers to maintain your information at the same level of confidentiality and security that we do. Some categories of third parties in which information is shared or disclosed include:
- Your employer as required to administer their benefits program
- Banks or financial institutions to process financial transactions and administer benefit accounts or program
- Health insurance companies to receive accurate claims
- Service providers that facilitate services of benefit programs
- Applicable law enforcement agencies, government authorities or other related third parties necessary to comply with legal or regulatory requirements
Flores may also share your personal information to affect, administer, or enforce transactions that are authorized by you. This includes, but is not limited to:
- To verify your identity per the USA Patriot Act
- To open and administer your accounts
- To help protect you and us from financial loss, or fraud
- To prepare account statements
- To prepare annual tax reporting information, if applicable
- To conduct risk management and verification to maximize the accuracy and security of your personal information
- To verify eligibility for benefits
- To integrate and provide our Services to you
When activating a plan with Flores, your employer may enable data sharing for the purpose of allowing their employees to view their own information on certain third-party and service provider websites, including our Websites. If your employer agreed to this, the categories of information shared or disclosed may include the same types of personal information listed in this Privacy Policy Notice.
Flores does not sell your personal data to third-parties and does not use your information for targeted advertising or marketing purposes.
Automatic Information Collection and Tracking
As you access or use our Websites, we may use automatic data collection technologies, like Cookies, to collect certain information about you. We use these cookies to improve the user experience. We do not use cookies for marketing or tracking purposes.
You may be able to disable cookies by activating the appropriate setting on your browser or device. If you select this setting, you may be unable to access certain parts or features of the Websites. Unless you have adjusted your settings to refuse cookies, our Websites will issue cookies when you access them.

Collection of Data for Minors
Flores does not purposely and knowingly collect the data of minors through use of our Websites. Our Websites are not designed or directed towards minors. For purposes relating to providing our Services to you, we may collect and process information about your dependents, who may be minors, with consent from you. You, as the parent or guardian, may opt-out at any time on behalf of the minor where appropriate.
Flores does not sell any personal information, including that of minors. Flores does not use personal information for targeted advertising and will not process the data of minors for this purpose.
Data Retention
Flores will store your information for as long as reasonably necessary to support the Services needed to administer your benefits, or as long as required by law or regulation, such as HIPAA.
Data Security
Flores is committed to the security of your personal information, and has implemented physical, administrative, and technical policies and procedures, as required or recommended by applicable laws, regulations, and standards, to safeguard your personal information. Our processes and procedures are continuously reviewed and improved to ensure we maintain strong security practices and protect you against fraud or unauthorized access.
We only collect information that is necessary to provide you with our Services and in some cases with cookies, to improve your user experience. When hiring a new employee, we restrict access to certain personal information such as based on an employee’s specific job function and allow only those who need access to provide Services to you or on our behalf to you. Flores encrypts all personal information provided to us on the Websites using Transport Layer Security. This ensures all personal information is encrypted in transit. However, this feature is only available if your browser supports this technology.
Flores requires that all users have a User ID and password to access certain online services and websites or portals. This is being done to ensure that you can be authenticated and access your own data. We recommend that you do not reuse an old password, and that you use a strong password to avoid unauthorized access. To further protect your account, you should enable two factor authentication. Do not share your password with anyone, and do not save the username and password for future use on a publicly available computer.

Per the Oregon Consumer Privacy Act (OCPA), Flores is required to include our name of which our business is registered with the Secretary of State, and the name by which we are known. We are registered and named as Flores & Associates, LLC, or otherwise named Flores.
